The Consultation and Collaboration Plan is the critical assignment for ECH 415.

The IFSP and IEP are two processes that require a generous amount of collaboration with many stakeholders.  The goal of this assignment is to assess your understanding of all the collaborative components involved in executing the IFSP or IEP process.

You have been provided a description and a blank form for each of these two processes.  You are NOT going to be filling these out, but you will need them to be familiar with each part, who participates, who is responsible for completing it, and how the follow-through might look.   You are going to pick one of the two processes.  A reminder that the IFSP is for children under the age of three and the IEP is for children over the age of three.  Because each of these processes are funded and organized by different entities (usually regional agencies created by the state for the IFSP and the school district for the IEP) they are each unique. 

Step 1:  Choose one of the two processes to focus your assignment on.  Research that process and be sure you are familiar with its components.

Step 2:   Review the brief scenario created for each of these processes.  Use the process/scenario to guide your responses specifically to each of the prompts below.

Step 3:   following headings and explanations:

Introduction: In your one-page introduction provide a brief summary of the IFSP or IEP process including its purpose and the importance of collaboration within the process.
Timeline/Sequence:  In one page, describe the timeline/sequence of this process. It is acceptable to use bullet points for this section in order to show the sequence of events that take place.  This should include how parents are communicated with and how they receive information and follow-up materials.
Stakeholders:  In one to two pages, describe who the potential stakeholders could be for the scenario associated with your process.  List who they might be and what their responsibilities are.  Be sure to specify which parts of the IFSP or IEP plan they are contributing to and how.  Include how the team lead will support and collaborate with stakeholders to ensure all responsibilities are fulfilled.
Communication and Collaboration with Family:  describe how the team will communicate and collaborate with the families, and what can they do to improve collaboration and ensure property delivery of information
Addressing Conflict:  share two potential conflicts that could arise and how they could be handled most effectively.
Conclusion:  Describe how collaboration affects the effectiveness of the IFSP or IEP process.

Scenario for IFSP –  Jeremy is a 15-month-old boy who lives 50% of the time with his mother and 50% with his father.  Jeremy was born at 40 weeks gestation with normal delivery.  Parents share that Mom struggle with mental health issues (depression).  They share that they fought a lot before they separated when Jeremy was 12 months.  They feel they both do their best to provide a safe and healthy home for Jeremy but they struggle financially and often need assistance with food and paying for rent in their individual homes.  At 13 months, Jeremy is still not walking and making very few sounds.  He babbles occasionally but has no words.  They have been referred to Early Intervention by their pediatrician who is concerned Jeremy’s development has not progressed.

Scenario for IEP – Shaina is a 4-year-old girl whose family just moved here from Guatemala.  She is the youngest of 4 children ranging in ages from 4-12 years.  They live in a 2 bedroom apartment and Dad works two jobs. Mom stays home to be available for the children.  Shaina’s parents have enrolled her in a local head start preschool which is her first school experience.  After a few weeks in the program, the director and teachers had a meeting with them where they shared some concerns regarding Shaina’s motor skills.  These concerns combined with her challenge of language (English is a second language), they suggest they have her tested with the school district.
