Description of the health problem in general – What it is and why it is an issue (health perspective, national prevalence, etc. can include “causes X # of deaths” and/or “can lead to…” etc.)? This is not the place for any local information. Use sources such as the American Heart Association, NIDDK, CDC etc. to get overview and statistical information.

Description of the community (demographics – age, race, culture, education level, % below poverty level, rural/urban, is the community growing or shrinking, other relevant information). Use sources such as your CHNA, local Chamber of Commerce website, and the US Census Bureau.

Description of the health problem within the community – it should be a problem that was identified by that CHNA as a priority for your community. Provide whatever local data you found, then use comparisons – local to state or national, or description of increasing rates. You should use this information to persuade your audience why this topic is especially of concern for this population.

What organizations within the local community will be involved in this campaign? Name organizations you would partner with within the local community to help ensure success of the campaign (non-profits, clinics, businesses, agencies, community organizations, churches).

What is the goal of the campaign?
What are the specific objectives? These need to be outcome objectives, and must be measurable. Hint: use and identify at least one specific Healthy People 2030 objective

Describe the model of behavior change that you will use to develop the campaign material that you will present. Then describe how using it will help you reach the objectives of the campaign.

Create or provide an example of material for your campaign (poster, video, meme, etc.) related to a general audience that serves to provide information about activities for or gives educational information about your chosen topic. This campaign material should help further your activities that can help you meet one or more of the objectives.
