CARE PLAN: Jane Bellini
Background:
Jane is a 26-year-old down syndrome individual. She lives with her mum and dad and 2 other siblings. She is currently engaged with her TAFE studies, trying to obtain a Certificate III in Horticulture. She would like to work in a garden nursery one day. She is of Italian descent and will tend to speak in Italian when she is upset.
Her goal is to be more independent and to have more social inclusion and have a wider social groups.
Communication:
Verbal: speaks both in English and Italian
wants to improve in her articulation of words
needs time to express herself
non-verbal:
at times, require pictograph
hand gestures cues maybe needed at times
positive facial expression
Vision:
magnifying glass needed when reading
eye drops at bedtime
Hearing aids:
left hearing aids
battery and volume need checking
Mobility:
Ambulant and weight bearing.
For long distance, may prefer to use a wheelchair.
X1 assist
Toileting:
Urine incontinence
Use day pad
Wrap around pad at night
Bowel:
Continence
Showering, dressing and grooming:
Able to shower and is independent but requires prompting to shampoo and other soap
Uses a shower chair and water needs adjusting
Able to take her clothes off
Mostly independent in grooming, needs supervision and prompting at times
Uses deodorant after shower and able to brush her hair
Require prompting at times to stop showering
Able to put on clothes with supervision
Prefer to wear pants, t-shirt, sweatshirt
Needs prompting to wash toes, nails and underarms
Has her full teeth; no dentures
Needs prompting to brush her teeth and uses mouth wash daily
Eating and drinking:
Food needs to be cut in tiny pieces
Doesn’t like hard food, prefer soft food; is on a semi-soft diet
Allergic to peanuts
Needs to be encouraged to drink minimum of 6 glasses of water
Likes ham and cheese sandwiches; likes pumpkin soup
Prefers to have breakfast after shower
Lunches given around 12.30pm no later than 1pm
Always have lunch in the kitchen table; dinner in front of the TV in the living room
Sleeping routines:
Sleeps at 2000H and prefers to have their door open
Wants a quiet sleeping environment
Needs 3 pillows and 2 blankets
Needs to have a cup of milk at 1930H
Wakes up at 0600H
If feeling grumpy in the morning, play instrumental music
Night checks every 2 hrs
Behaviour:
Can be verbally abusive in the morning
Can wave their hands at carers and can try to hit workers
Can be sarcastic and can insult staff when unsure about what they are doing
When told off, they can get more upset and cry
Had catastrophic behaviour in the past when forced to do things
Things to consider: speak politely, do not yell, use simple words
Keep the client in loop
Give time to calm down
Favourite things: plants, flowers, instrumental music
Comments:
Generally able to communicate and follow instructions
Can have behaviours of concern
Call parents and siblings when things can get out of control
Time off maybe needed
Q 1. Using the personal care support plan you have been provided with, discuss how you would go about determining the personal support requirements of the client, describe the strategies you would employ to maximise client participation, explain how you would provide personal support and address the workplace reporting and documentation you would involve in the process and answer must be specific to Jane’s care plan. (attached)
